· By purchasing and/ or using this voucher you agree to the conditions

· Only one voucher per person will be honored and Spa Visits are not transferable. You are welcome to purchase additional vouchers as gifts.

· When scheduling your appointment (s) please state the certificate number. We appreciate an advanced booking of at least 7 days. Earlier appointments may be available. Please note that this certificate is not valid during the month of December, however the bonus sections are available.

· Services cannot be redeemed the same day; each spa visit must be booked on separate days. However Essential Premium Bonuses can be book at any time including December.

· Please bring your Premium Spa VIP Voucher with you for each visit. The value of the services will be charged for if a valid voucher is not presented at time of visit.

· Please arrive 15 minutes prior to start time. Must be 18 years of age or have parental consent.

· In appreciation to the staff for exemplary service please tip at your discretion. Premium Spa VIP Vouchers do not include gratuity.

· Your scheduled appointment(s) are being reserved for you with our staff. If you are unable to keep your appointment, kindly provide at least 24 hours notice. We thank you for understanding that appointments cancelled with less than 24 hours notice will result in forfeiture of the scheduled service.

· This Premium Spa VIP Voucher will expire 6 months from date of purchase. It is your responsibility to allow sufficient time so as to enjoy all your Spa Visits.

· We cannot guarantee group bookings. Spa Visit appointments available Monday to Friday only. LOST OR STOLDEN CARDS CANNOT BE REPLACED

· The offers presented through the Premium Spa VIP Voucher are the sole responsibility of Zealous Medispa. Any questions or concerns regarding this certificate should be directed to Zealous Medispa Staff.

· We trust that you understand that the offers presented through this voucher are not valid in conjunction with any other promotion and that no substitutions will be permitted, that no refunds will be made for any reason whatsoever and that no verbal agreements will be honored, If you have heart concerns, or cancer 60 min massage or 60 min facial substitutions are available. If you are pregnant or become pregnant then certificate will be extended 6 month after breast feeding has ended. ORIGINAL VALUES: Based on approximations of regular retail prices.
